The Biological Truth: Why Is Everything About “Pressure”?

To understand the BBL rules, you must first understand the biology. When we purify the fat taken from your body and inject it into the buttock area, these fat cells need to establish connections with blood vessels in that area (Revascularization).

This connection process takes about 6 to 8 weeks. If you apply direct pressure to the buttock area during this time (for example, sitting on a hard seat during your flight or wearing tight jeans), you cut off the blood flow to that area. A fat cell without blood supply dies and is absorbed by the body. The result? Volume loss and asymmetry.

That is why the clothing and sitting rules I will explain below are vital not just for your comfort, but for the longevity of your investment in yourself here in Turkey.


Post-BBL Wardrobe Guide: What to Wear?

For the first few months after your surgery in Istanbul, your fashion sense needs to shift to “fat-cell-friendly” choices. Here is your stage-by-stage clothing strategy:

1. The Essential: Medical Compression Garment (Faja)

The special corset (Faja) we provide you immediately after surgery is your best friend.

  • What Does It Do? It compresses the edema in the liposuction areas (waist, back, abdomen), helps the skin re-adhere to the muscle, and supports your new body contour. However, notice that the buttock area usually has a special opening or a thinner fabric to avoid pressure on the fat.

  • Usage Duration: I generally require you to wear it for the first 6 weeks, 23 hours a day (removing it only for showers).

  • Tip for Travelers: When flying back home from Istanbul, ensure your faja is comfortable. We often use “lipofoams” (sponge sheets) under the corset to prevent creasing and ensure smooth healing during long periods of sitting/traveling.

2. First 6 Weeks: Loose and Comfortable Choices

This is the time to put away tight clothes.

  • Maxi Dresses and Shirt Dresses: These are ideal for your recovery, whether you are walking the streets of Istanbul or resting at home. They are easy to put on and apply zero pressure to the hips.

  • Loose Joggers: Sweatpants with a loose waistband and baggy hip area are perfect.

  • What NOT to Wear: Jeans, tight leggings, leather pants, and stiff fabrics with zippers are strictly forbidden. These clothes can irritate incision marks and crush the transferred fat.

3. Underwear Selection

This is a detail my international patients often overlook.

  • Seamless and Loose: Tight panties can dig into the “hip dip” area we just filled, causing indentations. Therefore, choose seamless, laser-cut underwear, or wear your underwear over your faja to avoid lines.


The Art of Sitting: How to Sit After BBL?

The biggest challenge after a BBL is sitting. I always tell my patients, “If possible, don’t sit at all for the first 2 weeks,” but we know you have a flight to catch from Istanbul to return home. Here are the rules:

1. The Golden Rule: Do Not Sit on Your Buttocks

For the first 2 weeks (and preferably up to 6 weeks), your buttocks must not touch any surface directly. You must always shift your weight to your thighs (the upper part of your legs).

2. How to Use the BBL Pillow?

This special pillow is a lifesaver, especially for your flight home from Turkey. It is made of firm foam and lifts your buttocks in the air by supporting your thighs.

  • Correct Usage: Place the pillow at the edge of the chair (or airplane seat). When you sit, the pillow should only be under your thigh bones (hamstrings). Your buttocks should hang freely behind the pillow, touching nothing.

  • Caution: To prevent leg numbness, you should stand up and take short walks every 15-20 minutes (or walk down the aisle on the plane).

3. Toilet Usage

This is one of the topics patients are most shy to ask about, but it is crucial.

  • How to Do It? Avoid sitting fully on the toilet. Instead, support yourself with your leg muscles in a “squat-like” position or use handles/walls for support. If this is difficult, you can place a soft towel on the front rim of the toilet (under your thighs) to keep your buttocks elevated.

4. Sleeping Position

Sleeping is just as important as sitting.

  • Prone (Stomach) Position: This is the safest and most ideal position. It puts zero pressure on the fat cells.

  • Side Sleeping: If you get tired of lying on your stomach, you can sleep on your side, but you must switch sides frequently.

  • Back Sleeping: Strictly forbidden for the first 6 weeks.


Recovery Timeline: Return to Normalcy

Here is the general calendar I give to my patients in Istanbul (though individual recovery may vary):

  • Weeks 1 – 2: “Zero Sitting” Policy. You can sit only for short periods for toilet needs or necessary travel (like your flight home). Otherwise, lie on your stomach or stand. Walking is encouraged.

  • Weeks 3 – 6: Modified Sitting. You can eat, drive short distances, or work using your BBL pillow. However, standing up and moving around every 30 minutes is mandatory.

  • Weeks 6 – 8: Transition to Sitting Without Pillow. You can slowly start sitting without the pillow. However, avoid hard surfaces (wooden chairs, concrete, etc.) and prefer soft cushions.

  • Month 3 and Beyond: You are free! You can wear your jeans and sit as you please. The fat cells are now permanent and part of your body.

I generally allow sitting without the pillow after the 6th week. However, this transition should be gradual. Initially, sit on soft sofas for short periods and avoid hard surfaces. If you feel pain or numbness while sitting, go back to using the pillow.

Q3: How should I sit while driving or flying home from Turkey?

For your flight home from Istanbul, you must use your BBL pillow. For driving, I do not recommend it for the first 2 weeks (due to painkillers and limited mobility). From the 3rd week onwards, you can drive by placing your BBL pillow on the driver’s seat and keeping your back straight.

Q4: My Faja (Corset) is too tight, can I loosen it?

The corset should “hug” you but not “suffocate” you. If it is tight enough to make breathing difficult or leave deep marks on your skin, it can impair blood circulation. Q5: My neck hurts from sleeping on my stomach constantly, what can I do?

This is very common. Placing a pillow under your chest or hips while lying on your stomach can relieve your spine. Also, sleeping in a semi-side position with a pillow between your legs is an acceptable alternative, as long as there is no direct pressure on the buttocks.
